APPLICATION PROCEDURE (New Students/Transferees)
STEP 1. Fill out the Application Form for Admissions.
STEP 2. Present and submit the accomplished form to the Admissions Office.
STEP 3. Pay the non-refundable Application and Testing Fee of Php 100.00 to the Cashier.
STEP 4. Submit the following requirements to the Admissions Office:
STEP 5. Present the Application and Testing Fee receipt to the Admissions officer to secure the ENTRANCE TEST PERMIT.
STEP 6. Take the entrance exam on the scheduled date.

3. Parents of accepted applicants should confirm enrollment on the dates indicated in the acceptance letter.
ENROLLMENT PROCEDURE (New Students/Transferees)
STEP 1. Claim the Clearance for Confirmation (issued only to applicants with complete submitted/presented requirements) from the Registrar’s Office.
STEP 2. Submit the accomplished form to the Admissions Officer and proceed to the Cashier for the Tuition and Fees Assessment.
STEP 3. Pay the Enrollment Fees depending on the chosen Terms of Payment (Full Payment, Semestral, Monthly, or Flexible).
STEP 4. Payment methods include Cash, Bank Deposit, or Check payable to:

a. Post-dated checks are not accepted.
b. The authorized signatory must sign any check alteration.
c. The following information shall be written correctly at the back of the check:
e. Replacement for returned/dishonored checks shall be paid in cash to the Cashier only.

STEP 1. Fill out the Application Form for Admissions.
STEP 2. Present and submit the accomplished form together with the following requirements to the Admissions Office:
STEP 3. Proceed to the Cashier for the Tuition and Fees Assessment.
STEP 4. Pay the Enrollment Fees depending on the chosen Terms of Payment
(Full Payment, Semestral, Monthly, or Flexible).
STEP 5. Payment methods include Cash, Bank Deposit, or Check payable to:

- Form 138 or Report Card (Certified True Copy)
- Certificate of Good Moral Character (Grades 4 - 7)
- Four (4) pcs. 1 x 1 ID picture
STEP 5. Present the Application and Testing Fee receipt to the Admissions officer to secure the ENTRANCE TEST PERMIT.
STEP 6. Take the entrance exam on the scheduled date.
TESTING GUIDELINES
- The set of tests shall determine proficiency in various learning areas.
- Be at the designated venue at least fifteen (15) minutes before the test. (Latecomers will not be allowed to enter the Testing Room once the examination has started.)
- Present the Test Permit to the proctor.
- Bring the following:
- Sharpened Mongol pencils #2 and eraser
- Snacks
- Jacket/sweater
- Get the results of the examination at the Registrar’s Office on the date announced by proctor.
RELEASE OF TEST RESULTS
- Test Results may be claimed from the Admissions/Registrar’s Office on the date announced by the proctor.

- Photocopy of Certificate of Scholarship (for scholars)
STEP 3. Proceed to the Cashier for the Tuition and Fees Assessment.
STEP 4. Pay the Enrollment Fees depending on the chosen Terms of Payment
(Full Payment, Semestral, Monthly, or Flexible).
STEP 5. Payment methods include Cash, Bank Deposit, or Check payable to:
